REM Iowa Community Services is seeking a Program Services Clinical Coordinator (PSCC) to join the Host Home team. This position will coordinate the development and implementation of services provided to individuals receiving services and oversees the services provided.
REM Iowa is seeking a PSCC to assist with conducting assessments for potential clients to determine if they would be a match for mentors in the Host Home model. PSCCs assess the ongoing need for therapeutic services for client in a Host Home setting by coordinating the development, review and revision of programs and Care and Support Strategies, providing , monitors and assures that comprehensive support services and oversight are provided on an on-going basis, schedules and/or provides opportunities for recreation and leisure activities with mentors and clients, maintains current and complete client reporting records, assists with pre-service, in-service and monthly support group. PSCCs may also assist in marketing the Host Home program.
Requirements:
- Bachelor’s degree in a human service related field and one year of experience working with individuals with intellectual disabilities; an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
- Knowledge and understanding of HCBS rules and regulations
- Excellent communication, presentation and writing skills; ability to display professionalism and confidence while giving presentations to small/large groups.
- Ability to build positive relationships and establish rapport quickly
- Strong customer service orientation - established level of patience, attention to detail, and strong follow through
- Current driver's license, car registration and auto insurance; reliable transportation for on the job use
