Responsibilities:

The Program Manager, Community Health/Population Health. will work Full Time, Days and serves as a subject matter expert in all aspects of the Population Health Department. This position is responsible for educating leadership team on other available Population Health and VBI programs, as well as keeping the team up to date on new expectations or clarifications on deliverables from current programs.

  • The Program Manager will have a variety of departmental level responsibilities including coordination of program teams, performance improvement, database management, analytic support, grant management, and program development.
  • The Program Manager will engage with both Hospital and Community Stakeholders in the planning, performance phases and programmatic deliverables.
Qualifications:
Bassett Healthcare Network follows the New York State and New York State Department of Health COVID-19 vaccine mandates for healthcare workers. Education/Qualifications:
  • Bachelor's Degree required.
  • Master's Degree preferred.
  • Associates degree with an additional 2 years of experience may be considered.
Experience:
  • Three years of healthcare management experience or management of regulatory programs required.
  • Master's Degree in Healthcare Administration or related field may be substituted for one year of experience.
Licensure/Certificates:
  • Valid NYS Drivers License required.
Skills:
  • Demonstrated leadership, management and solid interpersonal communication skills are essential.
  • Progressive experience in fostering interdisciplinary collegial relationships.
  • Ability to precept and mentor other staff members.
  • Strong analytic skills.
  • Strong computer skills required, specifically spreadsheet expertise (excel or access).
Physical Requirements:
  • Position requires extensive mobility with regular sitting and walking.
  • Ability to travel regularly within the region.
