Responsibilities

A successful Recovery Coach candidate will be an organized, efficient, proactive and responsive self-starter. A Recovery Coach engages and supports graduates of Landmark Recovery programs to improve clinical outcomes. The Recovery Coach is responsible for maintaining contact with graduates who have achieved alumni status through one-on-one coaching, monitoring of aftercare plan adherence, and application of outcome measures. Individuals that typically thrive in the role have a self-starter personality, excellent communication skills, and the ability to work effectively on a team.

  • Manage alumni caseload for two markets.
  • Assist alumni with transition from professionally directed treatment services to a self-directed recovery plan.
  • Assist alumni with development of recovery plan.
  • Monitor and track progress on recovery plan including adherence to aftercare appointments and participation in identified recovery supports.
  • Conduct regular one on one coaching sessions via telephone or videoconference based on support needs outlined in recovery plan.
  • Maintain documentation necessary related to coaching and supportive sessions.
  • Administer and track outcome measures to monitor recovery progression/regression.
  • Provide weekly report on outcome measures to Chief Clinical Officer.
  • Assign supportive curriculum based on changes to recovery status based on outcomes measure.
  • Provide crisis intervention as needed to support alumni to mitigate relapse risk and facilitate readmission to supportive services as necessary.
  • Participate in regular clinical supervision.

Qualifications

  • High School Diploma or GED required.
  • Minimum 2 years stable recovery from addictive disorder required.
  • Completion of CHW/CRS, CAPRC I, II, or NCPRSS is required

This position will begin as soon as a qualified candidate is selected.
