Job Requirements
- Valid OK or compact state RN license
- CPR Certification
- COVID-19 vaccination or exemption request submission
Job Duties
- Performs admission, recertification, resumption of care, discharge, supervisory aide and skilled nursing visits as scheduled by the Case Manager.
- Notifies the Case Manager if a visit cannot be made as scheduled. No visit will be rearranged without prior authorization of the Case Manager.
- Continually re-evaluates the patient’s nursing needs.
- Provides those services requiring substantial specialized nursing skills.
- Participates in case conferences to provide input into patient care.
- Performs complete and accurate OASIS documentation in a timely manner.
- Attends in-service training as applicable and required.
- Informs the physician and other personnel of changes in the patient’s condition and needs. Counsels the patient and family in meeting nursing related needs.
- Monitors and maintains productivity standards of self and team members and implements corrective measures when necessary.
- Coordinates and documents communication and services among the interdisciplinary team members.
- Maintains confidentiality of all patient records and office information.

Pay: $63,000.00 - $67,000.00 per year
