What does a Pinnacle Home Health RN do?
As a Home Health Nurse for Pinnacle Home Care, you will travel to a patient’s home to administer services and provide a variety of tasks during patient visits depending on the specific plan of care and type of patient. Some tasks include:
- Complete an initial assessment of patients and determine home care needs.
- Develops a care plan involving patients and their family.
- Medication/pain management
- Re-evaluate patient nursing needs.
- Initiate the plan of care and make necessary revisions.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Qualifications:
- Associate’s or bachelor’s degree with 1 to 2 years of recent acute care experience in an institutional setting.
- 2 years of home health experience preferred.
- Must be licensed in the state as a Registered Nurse (RN)
- Current CPR certification
- TB/PPD
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization’s requirements.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills and nursing skills per competency checklist.
