Envision Home Health and Hospice is seeking a full-time RN Case Manager who is passionate about providing quality care to patients and families in various residential settings including homes, assisted living facilities, and skilled nursing facilities within Snohomish County, WA, (Everett, Marysville, Edmonds, Lynnwood or surrounding areas).

$15,000 Sign On Bonus

This is with our new office in Snohomish and has the potential to move into a Director role! Great opportunity for the right candidate!

RN Case Manager responsibilities include:

• Coordinating all aspects of the interdisciplinary care and services provided to hospice patients and their families.
• Provides education to families and caregivers
• Develops, implements and reviews healthcare plans
• Advocating for patients and their families
• Documentation of care
• Nursing assessments
• Symptom management
• Promoting comfort and quality of life.
  

Qualifications:

• Current RN license to practice in the State of WA.
• Current WA driver's license
• Reliable transportation
• Maintain current CPR certification
• Excellent communication skills
• Minimum of one year nursing CM experience and one year experience in hospice work required.
• Self-directed and able to work with minimal supervision.
• Demonstrate excellent observation, problem solving, verbal and written communication skills, nursing skills per competency checklist.
• Shows ability to organize and prioritize workload independently.
