What We Offer:
PHC is currently seeking a Social Work Case Manager for a innovative new Permanent Supportive Housing facility to provide community-based, intensive case management services to individuals with complex medical and/or psychosocial needs who are also experiencing homelessness. Through our partnership with The Santa Clara County Office of Supportive Housing, we provide supportive services to ensure stability, self-sufficiency and overall improvement in quality of life to individuals that have been provided a permanent supportive housing subsidy.

Some of the job duties are:
- Manage assigned caseload as point person on interdisciplinary team.
- Provide in-person, community-based case management services that are strengths-based, client-centered and trauma-informed.
- Complete comprehensive bio-psychosocial assessment to evaluate needs and create appropriate care plan collaboratively with clients.
- In collaboration with the team, provide crisis intervention for immediate housing needs, medical, mental health and substance use issues.
- Act as an advocate for clients with social services, community-based agencies, health and behavioral health providers.
- Assist clients to function at most independent level possible by addressing ongoing psychosocial needs once clients are stabilized and not in crisis situations.
