Position Description: The System of Care Case Support Specialist is responsible for providing case support services to facilitate the organizing of care by supporting the service recipient in securing personnel and other resources needed to carry out all required care activities in the least restrictive, available, and disruptive environment for the enrollee. Duties and Responsibilities Administrative care coordination functions may be carried out by non-licensed staff working in a consultative role with the clinical care coordination staff. Administrative functions may include but are not limited to addressing additional support services and resources, assisting enrollees with arranging appointments, educating enrollees about other available supports as recommended by clinical care coordinators, and making phone calls to monitor enrollee attendance in treatment.

Responsibilities/Job Duties:

The System of Care Case Support Specialist is responsible for (though not limited to):

Collaboration – Leadership in collaboration. Represent the MCO in the Local, Regional and State Community Collaborative meetings. Promote consumer involvement and leadership in team meetings. Monitor consumer teams to ensure fidelity of the System of Care model. Ensure inter/intra agency collaboration to meet the needs of consumers and their families. Serves as a collaborative partner in identifying system barriers and solutions, through work with providers, stakeholders, families and advocates. Provide guidance and direction of collaboration in System of Care process.

Training and Development – Provides System of Care training and technical assistance with emphasis on providers and stakeholders within the MCO network that work with the designated population. Develops and presents educational resources to promote System of Care within the MCO provider network. Ensures that required documentation of System of Care activities is maintained and reported to state agencies in a timely manner.

The System of Care Case Specialist will work closely with the Licensed Care Coordinator to secure and monitor support services to assist the recipient in achieving the individual goals outlined in the plan of care. Facilitate coordination, communication, and collaboration with service recipients, providers, ancillary services, and others in order to achieve goals and maximize positive outcomes based upon individual assessments of needs and individual care plan.

Employee has regular contact with care provider and care recipient to assure ongoing access to care and follow through with plan goals. Assists with identifying and coordinating natural support systems; Promote Recipient self-determination through advocacy; Assist Recipient with development of natural support system; Facilitate and support Recipient informed choice, consent, and decision-making; Promote the Recipient’s self-care management; Focus on facilitating Recipient self-advocacy, education, and anticipatory guidance; Promote optimal Recipient safety; Promote the integration of behavioral change and principles; Link with community resources; Assist with navigating the health care system to promote effective care particularly during transitions;

Education/Experience/Licensure: Graduate of an accredited college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelor's degree accumulated Child Mental Health experience; or graduate of an accredited college or university with a bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated Child Mental Health experience.

Must reside in state of North Carolina.
