Primary duties include maintaining a high volume caseload, providing case management services through planning, monitoring and referral to assist individuals on their path to self-sufficiency and employment.

Serenity Targeted Case Managers will:

• Meet with clients in group and individual settings, preparing and implementing individual preferred lifestyle plans as a path to skills and independence.
• Provide support to Intellectual Developmental Disability Waiver recipients by assessing strengths and individual issues as it relates to participants goals regarding preferred lifestyle settings, community engagement and employment related activities.
• Conduct and monitor individuals services and referring participants to community resources and providing follow up as a part of case management service.
• Review SMART goals and assists participants in working toward goals opportunities within the community.
• Monitor and review the manner in which service delivery activities being conducted as the participants path to skills and preferred lifestyle by offering encouragement and suggestions for improving day and residential related activities.
• Be required to accurately report data regarding any related activities of participants to the state of Kansas.

Desired Skills and Expertise:

• Ability to perform comfortably in a fast pace, deadline oriented work environment.
• Experience working in IDD field with an understanding and knowledge of family structures.
• Ability to successfully monitor up to 30 participants fulfilling 100% of their TCM service guidelines.
• Ability to work independently and effectively within program goals.
• Ability to work a total of 8 hours a day between the operating hours of 7am-7pm. Monday-Friday
• Effective communication skills in both writing and verbal.
• Effective organizational skills.

Education and/or Experience:

Bachelor’s degree (BA) or equivalent from college or technical school; or six months plus related IDD experience; or equivalent combination of education and experience.

Salary: $30,000.00 to $35,000.00 /year starting wages, ability for increase based off seniority and length in position.
