Job description

At Home Solutions, is looking for fee-for-service UAS Assessment nurses to assess patients in the New York area. These assessments will be performed on behalf of select Managed Long Term Care (MLTC) plans, as well as our own waiver cases. Always following the standard UAS procedures and protocol.

Responsibilities:

• Perform a comprehensive MLTC in-home assessment
• Determines member eligibility for the MLTC program and qualifications.
• Develop and update individualized Person-Centered Service plan, based on a comprehensive assessment of members.
• Review and assign all incoming assessment requests
• Track and submit all assessments including supporting documents in accordance with New York State and Agency regulations.
• Conduct in-person, virtual, and telephonic assessments of members utilizing the UAS assessment Tool, for functional status, medical, behavioral, psychological, and community resource needs.
• Provides education/coaching in self-management of disease processes.
• Update member case records, and medication reconciliation.
• Conducts monthly outreach to check on members’ status.
• Data entry into the HCS system of information gathered during the visit

Requirements:

• Current and Valid NYS RN License
• UAS experience needed
• Liability insurance required
• Must be fluent in both English, Russian, and Spanish (other languages will be considered)
• Intermediate ability to use computerized systems such as Microsoft Outlook, Microsoft Word, and Excel.
• Excellent communication skills, detail-oriented, patient care, and assessment skills
• Ability to work independently
• Experience working in a home care environment or in an acute or sub-acute or long-term care (LTC) setting or managed long-term care (MLTC).

Pay: $5,250.00 per week
