Duties/Responsibilities:
- Oversees the daily operations in Care Coordination driving results that meet or exceed internal key performance indicators and service level agreements in value-based contracts
- Leads and directs operations team to drive efficiency and quality care outcomes.
- Ensures Care Coordination services are aligned with the Compliance and Quality Programs.
- Ensures staff practices are aligned to codified standard operating procedures; design and refine SOPs, as needed
- Design staff KPIs and drive performance accountability
- Maintains an organizational culture of positivity, motivation, and inclusivity.
- Sustains communications that promote alignment with organization's mission, vision, values and growth.
- Identifies training and continuing education needs and ensures proper training and ongoing education is developed and provided.
- Drives workforce development and retention. Develops leaders at all levels of the organization.
- Successfully integrates new growth opportunities into operations.
- Participates in developing and implementing contracts with payors, providers, healthcare systems and vendors in support of value-based care.
- Participates in the budget process to ensure operations are within approved budget.
- Collaborates with other areas of the business to carry out the organization's goals and objectives.
- Establishes and maintains collaborative working relationships with employees, internal business partners and stakeholders, senior leaders, and multiple healthcare system executives, as needed.
- Presents monthly and ad hoc performance reports and metrics to the Chief Clinical Officer, business partners and senior leadership, as needed.
Required Skills/Abilities:
- Ability to work in a fast-moving (and sometimes ambiguous) environment, while managing toward clarity and solutions
- Highly effective verbal and written communication skills; effective change management skills
- Ability to exercise a high degree of initiative, sound judgment, discretion, and decision-making skills
- Ability to solve management issues and direct numerous and varied operations simultaneously
- Ability to set and maintain high standards of performance, hold people accountable for achieving deliverable metrics, encourage team participation, and motivate others through a shared vision
- Knowledge of human resource management practices
- Knowledge of related regulatory and compliance requirements
- Possesses and maintains a current knowledge of healthcare industry operations and trends
Education and Experience:
- Ten years' experience in progressive leadership experience in healthcare delivery, minimum of 5 years' experience at advanced leadership level
- Clinical background preferred
- MBA or equivalent work experience preferred
- Experience with value-based care, Medicare / Medicaid / Managed Medicaid Long-term Services and Support preferred
Physical Requirements :
- Minimal lifting, bending but role typically requires long period of sitting
- Home office setting, indoor, extensive use of computer
- Must be willing to provide support and be accessible across multiple time zones
