Behavioral Health Assistant
Full-Time (3- 12-hour shifts Mon-Fri: 8am-8pm)
Looking for a Behavioral Health Assistant for our New Carlisle Community Health Center located in New Carlisle, Ohio. The Behavioral Health Assistant provides education and assistance to health center patients through supporting health educators.
DUTIES AND RESPONSIBILITIES:

Looking for a Behavioral Health Assistant for our New Carlisle Community Health Center located in New Carlisle, Ohio. The Behavioral Health Assistant provides education and assistance to health center patients through supporting health educators.
DUTIES AND RESPONSIBILITIES:
- Promotes effective and appropriate use of integrated health services at HPWO as part of a “Patient Centered Medical Home” (PCMH) model of care.
- Assist health educators in health care setting in all aspects of program implementation including coordination and data collection.
- Collects detailed data and enters in project data base.
- Report outcomes, troubleshoot data collection and other activities as needed.
- Comprehensive understanding of substance use screening, brief intervention and referral to treatment.
- Provides administrative support to program including entering contacts in database and other duties as assigned by program leadership.
- Communicates professionally, appropriately and clearly both verbally and in writing.
- Communicates and reports accurately and in a timely manner, meeting designated deadlines.
- Participates in training as it relates to program.
- Demonstrates and understanding of the needs of the underserved and vulnerable populations and abides by privacy and security standards
QUALIFICATIONS:
- Bachelor’s degree in healthcare profession, public health or social services preferred with experience in healthcare and/or community based settings. Minimum of Associates degree in related field.
- Ability to effectively manage projects.
- Good problem-solving and analysis capabilities.
- Must be detail oriented and possess organizational skills and accuracy.
- Ability to prioritize well and maintain appropriate sense of urgency.
- Excellent interpersonal, oral and written communication.
- Ability to work independently as well as part of a team.
- Demonstrate ability to relate to diverse populations.
- Proficient with Microsoft Office.
- Recognizes legal obligations to confidentiality of health care and program records
