Information Visit & Admit Nurse

For over 35 years, The Hospice of Baton Rouge has been meeting the end-of-life care needs of individuals throughout our 7 parish area and is the oldest continuously operating non-profit hospice provider in Baton Rouge.

The Hospice of Baton Rouge is seeking a Full Time Information Visit/Admit Nurse to join our growing team. This position works 7 days on and 7 days off. The hours are Friday - Sunday from 10:00 am - 10:00 pm and Monday - Thursday from 12:00 noon to 10:00 pm.

Qualifications:

· Minimum of 2 years experience as a Registered Nurse in either an acute care or home care setting

· Current Louisiana RN Licensure

· Current CPR Certification

Responsibilities:

· Maintain professionalism in all interactions with staff, patients, families and remember you are a representative of this organization.

· Conducts Informational Visits educating potential patients and patient’s family member on the hospice concept and services offered.

· Follow The Hospice of Baton Rouge’s Standard Operating Procedure for admission.

· Completes an initial, comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness.

· Admits patient to hospice program if patient meets admission criteria.

· If patient is admitted in a nursing facility, consents will be witnessed by a family member and nursing facility representative. Admit nurse will bring a THBR 3-ring binder to nursing facility for documentation. Initial Plan of Care to be left in binder along with copies of patient’s consents. Letter on billing procedure is delivered to facility billing office by admit nurse.

· The initial plan of care will be established on the day of admission by the admit nurse.

· Provides professional nursing care by utilizing all elements of nursing process.

· Assesses and evaluates patient’s status.

· Uses health assessment data to determine nursing diagnosis.

· Uses the nursing process to develop an initial nursing care plan in accordance with established policies and standards of care.

· Counsels the patient and family in meeting nursing and related needs.

· Provides health care instructions to the patient as appropriate per assessment and plan.

· Anticipates and initiates nursing interventions to prevent complications and alleviate symptoms of physical and psychological discomfort so an optimum quality of life may be maintained.

· Anticipates and meets emergency or crisis situations both physical and emotional.

· Teaches the patient and family re: the plan of care, its goals, and the implications of the diagnosis and prognosis.

· Maintains records, reports and forms that are accurate, same day charting/documentation or by 8:00 am following day (completed and synced), and contain quality content per documentation policies.

Pay: $32.00 - $36.00 per hour
