This individual will be responsible for reducing or eliminating patient psycho-social stressors which create barriers to effective medical cares and create patient-focused, strength and recovery-based medical social services. You will be a valuable resource to patients as well as clinical staff!

This position requires driving and serving clients in the Des Moines area as well as assisting with clients in our North Central areas (out of our Webster City office). Mileage is paid and additional drive time calculated into productivity!

• 
  Masters in Social Work from a school of social work accredited by the Council on Social Work Education
• One year of social work experience in a health care setting
• Valid Iowa driver's license
• Knowledge of nursing care in accordance with regulations and practices established by the Iowa Board of Social Workers
• Ability to assess client condition and implement plan of care, meeting physician order and client needs
• Ability to maintain valid professional licensure
• Excellent communication skills
• Previous experience in social work
• Knowledge of community resources
• Strong case management skills
• Previous experience with geriatrics and/or persons with mental illness is preferred
