Shift available: Double Weekend 6:00am - 10:00pm Saturday & Sunday

CC Young, a vibrant senior living community and a non-profit leader in senior living and Health Care is seeking a Licensed Vocational Nurse (LVN) for Assisted Living/ Memory Support to join our team! As a LVN you will provide quality nursing care to residents while following physician's orders, care plans, company policies and Nursing Best Practices.

Job duties include, but are not limited to:

• Prepares and reviews medical records and nurses notes.
• Monitor resident's activities to evaluate and provide nursing care in accordance with professional standards.
• Coordinates and assists with new admissions, transfers, and discharges.
• Communicates with residents, family members, interdisciplinary team members and management regarding resident status.
• Transcribe, prepare, and administer medication and treatment as ordered by physician.
• Assists with and participates in development and implementation of written care plans.
• Ensures the accurate and timely documentation of all resident treatment, medication, and other pertinent information.
• Performs other duties as assigned.

Minimum qualifications include:

• Must be a Licensed Vocational Nurse (LVN) in good standing and currently licensed in State of Texas.
• Minimum of 1 year of recent experience as an LVN, preferably in memory care, skilled nursing and/or hospice environment.
• Experience working with dementia care residents is highly preferred.
• Knowledge on the use of Sit to stand lifts is preferred but not required.
• Excellent verbal, written and interpersonal skills.
• Proven computer skills for performing extensive patient documentation/charting.

Pay Range $24.00 - $28.00
