Elizabeth Layton Center is looking for a full-time
Patient Admissions Navigator. This is a Monday - Friday position with hours of 8:30-5pm located in Ottawa, KS. EOE

SUMMARY: The
Patient Admissions Navigator is responsible for collecting, managing, and reporting required data related to CCBHC required outcomes and measures which include National Outcome Measures (NOM's) and required reporting measures for CCBHC. This requires both interpersonal and data management skills. The
Patient Admissions Navigator must be able to work collaboratively with clients, families, internal programs, external providers, and all aspects of the CCBHC network. The Patient Admissions Navigator must be an outcome focused individual that seeks to help clients have an effective and efficient experience upon admission to the Elizabeth Layton Center. The position reports directly to the Director of Operations.
MAJOR JOB RESPONSIBILITIES:
- Collects and completes National Outcome Measures (NOMS) questionnaires and other relevant data
- Enters NOMS data into the SPARS system in a timely manner
- Completes required patient intake documentation
- Completes all patient health measurements, e.g. blood pressure, height, weight
- Data entry and monitoring information into spreadsheets used to track information
- Monitors assigned CCBHC outcomes and coordinates with internal and external providers to increase frequency of desired outcomes
- Cross-trained in intake duties providing back-up throughout the day if necessary
- Maintains confidentiality of all appropriate employee and client information
- Ensure patients are fully informed about their legal rights
- Completes other duties as directed
- Complies with all ELC policies and procedures
EDUCATION and/or EXPERIENCE:
- Graduation from High School or the equivalent, some college preferred
- Ability to successfully complete required SPARS training
- Minimum 3 years clerical or general office experience/training required, medical office experience a plus
- Must possess a compassionate attitude and present a professional demeanor
KNOWLEDGE, ABILITIES, AND SKILLS:
- Ability to communicate effectively orally and in writing
- Ability to perform tasks accurately, effectively and in a timely manner
- Ability to perform basic mathematic calculations accurately
- Ability to understand and follow oral and written instructions
- Ability to establish and maintain effective working relationships with other employees, officials and the public
- Ability to be pleasant, friendly, and helpful to staff and visitors at all times
- Willingness and ability to support a diverse population of patients
- Ability to safely perform safely essential job functions with or without reasonable accommodations
