The Patient Care Specialist will coordinate the delivery of health care services to patients at our Venice Family Clinic in Inglewood, CA. As a Patient Care Specialist, you will:

• Provide administrative support to medical providers and medical staff.
• Discharge patients after medical visits and arrange for follow-up appointments
• Process referrals for Medi-Cal, Medicare, Managed Care and the County.
• Request authorizations using the Web Portal and E-consult.
• Perform data collection and perform extensive patient tracking and follow-up care.
• Assist with the training and supervision of volunteers.
• Perform other duties as assigned.

Required:

• Skill at coordinating the delivery of health care services to patients in a primary health care setting.
• Strong interpersonal skills to work effectively with people of diverse cultural, educational, economical, and social backgrounds.
• Strong written and verbal communication skills in both English and Spanish.
• Ability to prioritize and multi-task assignments with competing demands.
• Excellent organizational skills to complete work in a timely and efficient manner.
• Ability to work with minimal supervision, and ability to triage medical problems.
• Ability to work evenings as needed.
