Service Coordinator

We are seeking a self-starter to join our growing team of professionals specializing in geriatric care at the SeniorSelect Center. This is your opportunity to be an important part of a new model of integrated, value-based care and services for seniors and grow your expertise working alongside two industry leaders—National Church Residences and Central Ohio Primary Care (COPC).

In this role, you will be responsible for guiding seniors through our model of specialized, well-coordinated and comprehensive care and services. SeniorSelect Guides are integral to ensuring seniors reach their HealthCare and LifeCare goals by helping them navigate the complex healthcare landscape, manage social determinants and engage in meaningful educational programming and activities. Education: High School Diploma or GED required. Bachelor’s degree in social work or a related field is preferred.

Experience: Two or more years of experience in a social service delivery with elderly and/or family population. Demonstrated working knowledge of supportive services and other resources in the area served by the program. Demonstrated ability to advocate, organize, problem-solve, and provide results for the clients served.

Travel: Occasional

Pay: $20.15 - $33.58 per hour
