Summary
Under the direction of the Coordinator of Vocational Services assists in assessing the needs of the individual with disabilities and assist them to reach their maximum vocational potential.
Job Description:
- Develop Individual Habilitation Plans (IHP) and coordinate IHP meetings.
- Monitor and communicate the progress of the individuals with disabilities to parents, guardians, sponsors and other agencies.
- Provide one- on-one training/job coaching and assist the supervisor with any issues that may arise in the work area.
- To observe All the clients in their work environment at least one time per week
- Develop and supervise downtime activities and lunches.
- Identify services the individual with disabilities may need and make recommendations/referrals to other programs/services.
- Oversee the proper administration of medication, including compliance with all procedures and recordkeeping.
- Insure immediate access to emergency services in the case of life threatening emergencies in full compliance with Danielle’s Law.
- Provide individual and crisis counseling.
- Provide evening coverage for emergencies
- Contact appropriate people when needing to report an incident (UIR)
- Generates timely and accurate reports on the status of program and individual progress.
- Assists with caseload coverage.
- Occasionally may need to transport individuals with disabilities to and from their residence, hospital/workers’ compensation visit and to and from various work/contract assignments using their own vehicle when an OTC vehicle is not available.
- Performs other duties as assigned.
- Maintain yearly certificates for trained deemed mandatory by OTC. Such as: Blood Borne Pathogens, and Supportive Employment.
Requirements:
- BA or BS in a Human Services, Psychology, or Vocational Rehabilitation services.
- Minimum 1 year experience working with individuals with disabilities.
- 1 Year Job Coaching or Employment Specialist required.
- 21 years or older.
- The successful completion of all DDD required training as defined in the DDD SE/DP Manual and compliance with all applicable standards as set forth in the manual upon the date of hire.
- Acceptable Background Check and Drug Test.
- Valid driver license with an acceptable driving record as set by OTC’s Insurance Carrier.
- Essential to have reliable transportation and maintain required state and/or OTC vehicle insurance coverage.
- Ability to perform CPR and First Aid on ground level.
- Availability 11am to 7pm, hours may vary.
